A group of health professionals is ready to give you the best care possible while saving you time, money and stress. Meet your care team’s most valuable players.

When you think of the health care workers in your life, your primary care provider (PCP) is probably the first person who comes to mind. But so many more people in the health care world can play a big role in managing your chronic health condition. 

 
These professionals can help in ways that go well beyond giving you the checkups and treatments you need. For example, you might need help getting healthy foods, transportation to health care appointments or at-home medical supplies. Getting access to these resources can play a major part in your physical and mental health.  

That’s why as an Aetna Chronic Condition Special Needs Plan (C-SNP) member, you have an entire personal care team dedicated to you. Their job is to save you time and stress while helping you stay as healthy as possible. They can: 

  • Guide you through your health plan so you can get the most out of your benefits 

  • Work with you to create a care plan that meets your personal needs and goals 

  • Put your care plan in motion 

  • Jump into action if you have a problem that may threaten your well-being 

Here are the MVPs who may be on your care team, plus other Aetna professionals who are ready to support you. 

Health survey specialist

This care team member’s main job is to help you complete your annual health survey. They’ll ask you a few simple questions about yourself, your health and your medical history. Your answers help your care team understand how to best support you and make sure you’re taking advantage of the right benefits and services.  

If you haven’t gotten a call yet, you can call the number on your member ID card to complete your survey. 

Care manager

Once your health survey is filled out, it’s sent to your care manager. They are a registered nurse or another health professional that best fits your health needs. They’ll review the information and then give you a call. During that conversation, you’ll be able to talk about which areas of your health need the most attention. They’ll also ask you about your health goals and challenges.  

 

After that, the care manager can create a full care plan that’s tailored to your health needs. They’ll work closely with your PCP to put this plan into action. 

 

For example, your care manager might notice that you need help managing a condition like diabetes or heart disease. But maybe finding long-term housing is your biggest concern right now. After talking with you, your care manager can decide what to focus on first. They can also bring in another care team member for extra support, like a social worker or pharmacist.

Find an in-network provider near you

Use the Aetna® provider finder tool to search for doctors, specialists and other providers in the network. It’s an easy way to find the care that fits your health needs.

Social worker

A social worker can help with basic needs that affect your health. That may include finding safe or permanent housing, paying for utilities or getting healthy foods. The social worker can answer questions about social services, such as food assistance and affordable housing. They can also connect you with the right programs. 

Care coordinator

A care coordinator knows the health care system inside and out. This expert understands all the C-SNP benefits and can find the best way to help you use them.  

 

Care coordinators play a large role in easing your burden. They navigate complex systems and services for you. For example, they may be able to help you: 

 

  • Schedule a medical appointment
  • Arrange transportation to your provider’s office
  • Get authorization for at-home medical equipment, medications and testing

Pharmacy team

At least once a year, a pharmacy team will review your medications. Then they’ll make recommendations to your PCP or care manager about any medication changes that might benefit you.  

Behavioral health care manager

Chronic health conditions like diabetes or heart disease can sometimes cause stress, depression or anxiety. Or you might need help with something that happens occasionally, like grief or trauma.  

If you need mental health support, your care manager may bring in a behavioral health care manager. They can help you get the resources you need.
